Overview

Catastrophic impairments present a great challenge to an individual’s ability to maximize functional independence and preserve a good quality of life. A Life Care Plan developed by Harold Bialsky systematically addresses all of the short and long term needs of an individual with a severe disability in order to ensure their functional independence and quality of life.

What is a Life Care Plan?

A Life Care Plan is a dynamic document based upon published standards of practice, comprehensive assessment, data analysis and research. This document provides an organized and concise plan for current and future needs and includes associated cost for individuals who have experienced catastrophic injury or have chronic health care needs. The development of the Life Care Plan includes collaboration and communication with the client, family, caregivers, and medical providers.
